I hereby give my consent to participate in all activities of WAISN. I understand that the scope of my relationship with WAISN is limited to a volunteer position and that no compensation is expected in return for services provided by myself; and that WAISN will not provide to me any benefits traditionally associated with employment. I desire to engage in activities related to serving or participating in WAISN’s activities as a participant or volunteer. I am responsible for my own insurance coverage in the event of personal injury or illness as a result of participation in WAISN’s activities. 

1. Volunteer Status. As a volunteer at a WAISN event or for a WAISN program, I understand that I control the dates and times when I participate in WAISN’s activities. I also understand that I will not be compensated for any time spent attending, nor am I entitled to benefits. 
2. Miễn trừ và giải phóng. I release and forever discharge and hold harmless WAISN and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the activities as an attendee with WAISN, including claims arising out of negligence I understand and acknowledge that this release discharges WAISN from any liability or claim that I may have against WAISN with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services I provide to WAISN or occurring while I am a volunteer.  
3. Bảo hiểm. I affirm that it is my responsibility to be covered by primary medical insurance and understand that I am responsible for my medical bills and expenses if an injury occurs. Further, I understand that WAISN does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health or disability benefits or insurance of any nature in the event of my injury, illness, death or damage to my property. I expressly waive any such claim for compensation or liability on the part of WAISN beyond what may be offered freely by WAISN in the event of such injury or medical expenses incurred by myself. 
4. Vehicle. I understand driving a community member for an accompaniment is a profound responsibility and I will exercise extreme care and due diligence while driving. I understand that as a volunteer driver, I must be 21 years of age or older, possess a valid driver’s license, have the proper and current license and vehicle registration, and have the required insurance coverage in effect on any vehicle used for volunteer accompaniment. I will not make phone calls or text while operating a vehicle as a volunteer unless they are using a hands-free device. In Washington state, drivers can use hands-free devices while driving, but cannot hold the device to their ear or use it to text, type, read, or write. Hands-free devices must require only a single touch or swipe to activate a function. Using a device for navigation or other functions is allowed if it is hands-free and does not require more than a single touch to activate a function. If I do not have access to a hands-free device,  I will pull over to a safe place and come to a stop prior to any such use. If I am required to drive in my capacity as a volunteer driver, I agree to complete the WAISN Volunteer Driver Form. (See below.)
5. Giả định rủi ro. 6. Non-Interference with Law Enforcement. When volunteering as a WAISN rapid response volunteer or during a community accompaniment, I will not obstruct, resist, hinder, impede, or interfere with any immigration enforcement agent or police officer in the execution of their duties.
7. Harboring. When acting as a Volunteer for WAISN, on WAISN grounds, or at WAIS N events, I will not knowingly give assistance for the purpose of helping someone to violate immigration law, including but not limited to: sheltering known undocumented individuals to conceal them from the government or taking any action that conceals non-U.S. citizens from immigration agents, such as assisting them in fleeing or dodging immigration agents and warning someone known to be undocumented that immigration is present. 
8. Chính sách. I shall abide by all WAISN policies and procedures provided to me.
9. Phát hành ảnh. Tôi cấp và chuyển giao cho WAISN mọi quyền, quyền sở hữu và lợi ích đối với bất kỳ và tất cả các bức ảnh, hình ảnh, video hoặc bản ghi âm của chính tôi hoặc hình ảnh hoặc giọng nói của tôi do WAISN tạo ra liên quan đến việc tôi tham gia vào các sự kiện của WAISN, bao gồm nhưng không giới hạn ở , mọi khoản tiền bản quyền, tiền thu được hoặc lợi ích khác có được từ những bức ảnh hoặc bản ghi âm đó. 
10. Điều trị y tế. Theo đây, tôi từ chối và vĩnh viễn loại bỏ WAISN khỏi bất kỳ khiếu nại nào phát sinh hoặc có thể phát sinh sau này do việc điều trị sơ cứu hoặc các dịch vụ y tế khác được cung cấp liên quan đến trường hợp khẩn cấp trong thời gian tôi là người tham dự với WAISN. Tôi đồng ý cho WAISN cung cấp, quản lý hoặc điều trị y tế cho Tôi trong thời gian tôi tham gia các hoạt động của WAISN.
11. Indemnification and Legal Representation. I hereby agree to indemnify, defend, and hold harmless WAISN from any and all liability, losses, damages, judgments, or expenses, including attorneys’ fees, that it may incur or sustain as a result of my involvement in the Activities, arising out of any third-party claim. I shall hold WAISN and its board of directors, employees, and contractors harmless in the event of legal action, arrest, questioning, or threats by law enforcement or immigration agents as a result of my volunteer activities. I understand that WAISN will not provide or bear the cost of legal representation in the event I am subject to legal proceedings.
12. Không có bảo hiểm hoặc bồi thường cho người lao động. I understand that WAISN does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, disability insurance of any nature in the event of my injury, illness, or death, or damage to or loss of my property. I understand that if I am injured in the course of the activities, I am not covered by WAISN’s Workers’ Compensation program.

14. Khác.
    1. Tôi hoàn toàn đồng ý rằng Bản cho phép này nhằm mục đích mở rộng và bao trùm trong phạm vi được luật pháp của Bang Washington cho phép và rằng Bản cho phép cho phép này sẽ được điều chỉnh và giải thích theo luật pháp của Bang Washington. Tôi đồng ý rằng trong trường hợp bất kỳ điều khoản hoặc điều khoản nào của Xác nhận cho phép này bị coi là không hợp lệ thì khả năng thực thi các điều khoản còn lại của Xác nhận cho phép này sẽ không bị ảnh hưởng. 
    2. Bằng cách cung cấp cho WAISN địa chỉ email của mình, tôi đồng ý tham gia danh sách gửi thư của WAISN. Tôi có thể từ chối bất cứ lúc nào. WAISN không chia sẻ thông tin cá nhân.
    3. Tôi đồng ý rằng Bản cho phép này thể hiện sự hiểu biết đầy đủ giữa WAISN và tôi và thay thế tất cả các thỏa thuận, hiểu biết, tuyên bố và bảo đảm khác trước đó, cả bằng văn bản và bằng miệng, giữa chúng ta, liên quan đến chủ đề của thỏa thuận này. 
    4. Nếu bất kỳ điều khoản hoặc điều khoản nào của Cho phép sử dụng này bị bất kỳ tòa án có thẩm quyền nào coi là không hợp lệ thì điều khoản hoặc điều khoản đó sẽ được coi là sửa đổi để có hiệu lực và có thể thi hành trong phạm vi tối đa cho phép. Tính vô hiệu của bất kỳ điều khoản hoặc điều khoản nào như vậy sẽ không ảnh hưởng đến hiệu lực hoặc khả năng thực thi của các điều khoản và điều khoản còn lại.

    7. Bằng văn bản này, tôi đồng ý rằng Bản cho phép này nhằm mục đích mở rộng và toàn diện ở mức cho phép và rằng Bản cho phép này sẽ được điều chỉnh và giải thích theo luật pháp của Bang Washington mà không tham chiếu đến bất kỳ sự lựa chọn nào về học thuyết luật.
